WORK PERMIT INSTRUCTIONS

 

APPLICATION FOR MINOR WORK PERMIT FORM

The student will complete the top of the form, and the employer will complete the bottom portion. The form must be completed so the actual Work Permit can be processed. Once complete, bring this form to Mrs. Kidwell in the High School office. She will use this information to complete the actual work permit.

 

PHYSICIAN’S CERTIFICATE FOR MINOR WORK PERMIT

The employer requires this form; the school does not. If the student has a current physical on file with the High School. Please see Mrs. Landis in the Athletic Office for a copy if needed. Attach the physical to the Physician’s Certificate form. If the student does not have a physical on file, you will need to have this form completed by a physician.
